If you qualify for NJ Temporary Disability Insurance, you can complete the application online via this link: https://www.nj.gov/labor/tdi/tdiindex.html
Once you complete your online application you will receive an 11 digit "Online Form Id" which you must provide to humanresources@kippnj.org in order to complete the Employer portion of the application. Additionally, your doctor must also complete a section of the online application.
Once all section of your application have been completed, you can use the following link to check the status of your TDI application and payments: https://lwdsso.dol.state.nj.us/amserver/UI/Login?goto=https%3A%2F%2Fwww14.state.nj.us%3A443%2FDOL_DABI&arg=newsession.
Please note that the state will periodically send you documents in the mail. Be sure to complete them and send them back ASAP to avoid delays in you receiving your funds. Additionally, the TDI application and approval process can be a lengthy one, so please expect a 3 week minimum delay between when you submit all sections of your application and when you begin receiving TDI benefits (sometimes it can take much longer).
Once your application is approved, you will receive a Bank of America debit card from the state. Your funds from disability will automatically be forwarded to that card. Please keep this card in a safe place. Any future TDI payments from subsequent leaves will be deposited to this card.
