If you have not received your Optum Bank | HSA debit card or if your card has been lost or stolen, please reach out directly to Optum Bank, our HSA service provider, to request a new card to be mailed to you.
When requesting the card, please be sure to confirm the address Optum Bank has on file for you to ensure proper mailing. If the address they have on file is incorrect, use the path below to make the update to your mailing address using the ADP self-service system.
Updating Your Address – ADP Log into ADP > Myself > My Information > 'Click' pencil to the right of your name > Scroll down Personal Info window and update Address fields > Save
It can take up to 14 business days to receive the card in the mail. For security purposes, your debit card will arrive in a blank, white envelope with blue writing. This envelope often gets mistaken for junk mail but will contain your Optum Bank | HSA debit Mastercard.
